Fees are due each session unless a different arrangement has been made.
Payment is accepted via personal check, cash, debit card, Health Spending Account (HSA) card, credit card, or PayPal.
REDUCED FEE
Dependent upon your financial situation, I do offer services at a reduced fee.
If I cannot accommodate your financial situation, I will refer you to another provider.
INSURANCE
I am an out-of-network provider. I will provide you with a “super bill” (a monthly statement/receipt) that you can submit to your insurance company for potential partial reimbursement.
Please check with your insurance company about whether your plan will reimburse you for my services.
Below are the typical Procedure Codes (CPT Codes) that I use when working with clients and families:
Weekly Sessions = 50 minutes = $200; shorter sessions and/or consultations are prorated according to amount of service time
Initial diagnostic evaluation: 90791 (1st parent intake session)
Family therapy with client present: 90847
Family therapy without client present: 90846
Individual psychotherapy (30 minutes): 90832
Individual psychotherapy (45 minutes): 90834
Individual psychotherapy (60 minutes): 90837
Phone consultation (with school staff or other professionals):
98967 (15-20 minutes) and 98968 (30 minutes)
Collateral meeting with school staff: (blank) – Check with your insurance company to determine if this treatment procedure code is covered.
Place Of Service (POS) code: 95 should be a modifier code for insurance to indicate telehealth, as well as POS code of 02, 11, though this may vary with each company.
GOOD FAITH ESTIMATE
Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate.

CANCELLATIONS & MISSED APPOINTMENTS
It is expected that you and your child will attend each scheduled session.
If you need to cancel your scheduled appointment, you must do so more than 48 hours in advance of the appointment.
If you cancel your appointment with less than 48-hours notice (unless you are able to reschedule for a later time that same week) or do not show up for your appointment without any prior notice, then I might charge you a late-cancellation/no show fee.
Please be aware that insurance companies do not reimburse for missed appointments.
